Just one week after the untimely death of her plastic surgeon, Dr. Frank Ryan, Heidi Montag sat down exclusively with Life & Style to tell-all about her plastic surgery regrets, revealing that she wants her outrageous G implants removed.

“I’m desperate to go back to normal,” she confides to Life & Style. “I’m downgrading and going a little smaller, to a D or a double D.”

More than nine months after she went under the knife on Nov. 20, Heidi says she is still in severe pain and her body has not gotten used to the over the top boob job. “I have major anxiety about it. I was taking painkillers but they weren’t working so I stopped. It hurt either way,”

Besides being unable to hug her four dogs or wear anything but custom-made clothing, “I’m obsessed with fitness but it’s impossible to work out with these boobs,” she says. “It’s heartbreaking. I can’t live an everyday life.”

And while Heidi tells Life & Style she is currently shopping for a surgeon in South America, she worries that she’ll be trapped in this cartoonish body forever after the untimely passing of her confidant Dr. Ryan. “I feel trapped in my own body,” Heidi tells Life & Style.

“There’s just no fixing it,” she says. “Dr. Ryan knows the work he did, he knows everything.”

Just when you thought it was over, Speidi returns.

Although Spencer Pratt signed divorce documents in his split from Heidi Montag less than two weeks ago, the duo continue to stir up drama.

Pratt says he’s shopping a sex tape of Montag with another woman and also writing a relationship tell-all. Plus, the two have been sparring on Twitter.

But Sunday, they were spotted by TMZ.com together in Costa Rica where they are still spending time.

So what gives?

“I had to bring the puppies down and help them move in,” Pratt tells PEOPLE. “[Heidi] thought I was going to leave, but I’m not. I didn’t want to get divorced … I have nothing in Malibu, I’m homeless, I’m nothing without her.”

Montag admits she agreed to have Pratt join her in Costa Rica. “Everything was calm and going smoothly with the divorce,” she says. “We were really getting along and it was amazing he was being so thoughtful to help me move. Then things turned.”

By things turning, Montag is referring to the supposed deal Pratt is working on with Vivid Entertainment to sell a sex tape. “There is no sex tape that exists and there never was,” she says. “Spencer is trying to drum up press. This is exactly why I wanted to divorce him in the first place.”

But Pratt insists such a tape does exist. “I told Heidi … if you don’t want to do the new reality show with me, then I’ll have to release old footage,” he says. “I can get $5 million from a sex tape, but I really don’t want to do that. It’s not the way I want things to be. Heidi is furious with me but I need to look out for myself.”

As for the future, “I’m going to leave the hotel and get a house because Spencer won’t leave me alone and won’t go back to Malibu right now,” Montag says. “It’s just too hard. I still love him, but I can’t stand what he’s doing.”

Now that Heidi Montag has filed for divorced from Spencer Pratt, the Hills star is reflecting on the good old days she once shared with her former TV costars

“[I'm] watching old Hills [episodes],” Montag, 23, Tweeted Wednesday. “I miss you @AudrinaPatridge and most of all @laurenconrad.”

Montag, who recently Tweeted she “has no friends,” reportedly changed her phone number and has had no contact with any of her Hills cast mates (besides Pratt, with whom she shares custody of their dogs) since she finished filming the show.

Montag did not appear in the final episode of The Hills.

A rep for Conrad did not return a request for comment, and because Patridge is out of the country, her rep says she cannot be reached.

Almost two months after Heidi Montag went to court seeking legal separation from husband Spencer Pratt, the reality starlet filed for divorce.

Montag, 23, cites the usual irreconcilable differences in the divorce petition filed in Santa Monica, Calif., Superior Court.

“Heidi has amended her petition for separation and today has filed a petition for dissolution of marriage from Spencer Pratt,” Montag’s lawyer Jodeane Farrell tells PEOPLE. “The couple has agreed they would like their divorce to be finalized in a timely manner in an out of court settlement. Both parties are amicable with each other and over the possibility of finalizing their divorce.”

The public and Montag’s own former costars from The Hills initially expressed doubts about whether the split was legitimate or another publicity stunt.

They were married last April at a church in Pasadena, Calif., and have been busy ever since.

Montag underwent 10 plastic surgery procedures in one day this year. She followed that by firing Pratt as her manager and beginning her quest to become a big screen actress, filming a role in Adam Sandler’s Just Go With It, and also working on her music career.

Just recently, Pratt, 26, admitted to choosing fame over his wife telling PEOPLE, “We love each other but I’m a fame whore and I’ll never grow out of it. [Heidi] knows that and doesn’t want that.”

Heidi Montag will be appearing on one less reality show.

The newly single Hills star, 24, planned to pair up with her pal Jen Bunney on the small screen, but Bunney says those plans are off.

“Originally I was supposed to do a reality show with Heidi , but right now it looks like some things have changed,” Bunney — who left the second season of The Hills after feuding with Lauren Conrad over Brody Jenner — writes on her blog.

What’s more, she doesn’t know if “Heidi wants to be involved in reality TV at the moment with everything going on in her personal life (no we are not in a fight, and no Heidi is not “dropping” me to do reality shows with the twins).”

Montag is reportedly shopping around The Heidi Show with Hugh Hefner’s ex girlfriends, Kristina and Karissa Shannon.

“Since that recent development our original network and some others as well have offered to continue on with the show despite Heidi possibly not being involved, which I just recently agreed to,” she says. MTV is not one of the networks, according to her blog.

“Although, I of course wanted to pursue this project with her, I also respect that she has a lot to deal with right now and have to decided to go ahead and move forward with it alone, since these opportunities have presented themselves (my hope is that she will want to be involved later on),” says Bunney of her pal, who filed for legal separation last month from Spencer Pratt (even though sources have told Us Weekly they are “laughing” over their “fake split”).

Continues Bunney, “So I have already been to a few really productive meetings in the past few weeks, and the show is moving forward, looking to air this January. I was actually surprised that this happened, considering my name is not one that has been plastered across every tabloid the past few years, but I am super excited. I suppose the buzz from the original show with Heidi garnered enough interest and publicity and enough about me as an individual that several networks are interested in still pursuing it despite Heidi’s absence. So that’s the news for now!”

Spencer Pratt always has some strange new ploy for attention up his sleeve – but not his split from wife Heidi Montag.

That, he says, despite the fact that nobody has seen divorce papers and family and Hills castmates are skeptical, is the real deal.

“We love each other but I’m a famewhore and I’ll never grow out of it.” Pratt tells PEOPLE. “[Heidi] knows that and doesn’t want that.”

“I want every kind of press,” he says. “She believes in bad press. There’s no way my love for fame and her love for puppies will ever work out successfully. She just wants to hike and hang out and be calmer.”

As for their infamous “Speidi,” moniker, Pratt says his estranged wife “doesn’t want to be Speidi anymore. She wants to be Heidi Montag: the sex symbol.”

“She thought I’d burn out of this, but no, I’m still the same Spencer who went on The Hills to be famous,” he says. “I still need to do stunts and take cues from Brad Pitt and Tom Cruise.

Pratt, who mentions he and his ex are “friendly,” says Speidi trouble is nothing new. “It’s been a constant battle since we got married,” he says. “She would be like, ‘Are you really Tweeting that? Are you really doing that?’”

So what’s next for the limelight-loving reality personality?

When fighting cyber crime fell through, Pratt says he decided to grow a beard and turn to art. “I’m switching it up,” he says. “I’ve already gone for the blonde, spiky-haired look. Now I’m going for the Hollywood producer look.”

Continues Pratt: “I’m an artist now. I have an easel and everything. I’m going for an art show and a gallery.”
